Forms for Pre-Registration and Dormitory Reservation


Pre-Registration

Pre-registration is highly encouraged with a special rate for those prepaying on or before May 15, 1997. Payments or reservations received after June 9, 1997, will be processed as walk-ins. Cancellations must be received in writing by June 11, 1997.

Dormitory Reservation

The Ohio State Dormitories have adopted a reduced rate for those prepaying their accommodations by May 15, 1997. The prepayment and regular dormitory rates are indicated on the registration forms.

Pre-registration forms and dormitory reservation forms are available by FTP in ASCII, PostScript, WordPerfect6.0, WordPerfect5.1. (Better approach if you have a slow internet connection)

Alternatively, you may click to view a PDF (reader can be downloaded for Adobe Acrobat) picture of the registration form which can be printed directly from your browser. (Simpler approach if you have a fast internet connection.) Please fill out the printed form and mail it to us with your payment. We can only accept checks in U.S. funds drawn on a U.S. bank. Anyone who finds this requirement impossible to fulfill should contact the Symposium office directly.

We cannot accept credit card payment through the internet or in person. However, Drackett Tower will accept payment for dormitory charges only through MasterCard at check-in. (NOTE: This would be at the post-May 15 rate)

If you would like accommodations on Saturday night, June 14, call either the Holiday Inn on the Lane at 614-294-4848 or Ramada University Hotel at 614-267-7461. Identify yourself as being with the Molecular Spectroscopy Symposium to get the OSU discount. Room block will be released after May 14.

Refunds must be requested in writing by Wednesday, June 11. NOTE: Payments or reservations received after Monday, June 9 will be processed as walk-ins.


LIABILITY: The Symposium fees DO NOT include provisions for the insurance of participants against personal injuries, sickness, theft or property damage. Participants and companions are advised to take whatever insurance they consider necessary. Neither the Symposium organizing committee, its sponsors, nor individual committee members assume any responsibility for loss, injury, sickenss, or damages to persons or belongings, however caused. Persons wishing information on travel insurance while in the USA should contact private carriers.
